About the Title V CCS 5-Year Needs Assessment
Title V-funded program are required to complete a needs assessment every 5 years. The Family Health Outcomes Project at UCSF will be facilitating the Needs Assessment Process over the next 11 months. The CCS Needs Assessment and Action Plan Goals are:
• Within budget and legislative constraints, determine Action priorities to be addressed during FY 2010-2014
• Identify the most important and potentially effective changes CCS can make to improve services for CCS-eligible children
More information is available in the slideshow presentations from the Kickoff Meeting.

Key informant interviews will be planned and conducted September through November. Focus groups will be planned and conducted October through December. Web-based surveys will be planned and conducted January through February. In March, a second stakeholder meeting will be convened to prioritize needs. There are many opportunities to participate, and we hope you will be involved!
